Good Faith Estimate (No Surprises Act)

Under the federal No Surprises Act, health care providers are required to provide clients who do not have insurance or who are not using insurance with a Good Faith Estimate of the expected costs for services.

This notice explains your right to receive a Good Faith Estimate for mental health services provided by Best Life Counseling Services.


Your Right to a Good Faith Estimate

You have the right to receive a Good Faith Estimate explaining the expected cost of your mental health care.

This applies if you:

  • Do not have health insurance, or

  • Choose not to use your insurance for counseling services

A Good Faith Estimate provides information about the expected charges for services such as therapy sessions or related care.


What the Good Faith Estimate Includes

The Good Faith Estimate outlines the anticipated costs for services that may reasonably be expected as part of your care.

It may include:

  • Individual counseling sessions

  • Assessment or intake appointments

  • Ongoing therapy services

  • Any related mental health services provided by the practice

The estimate is based on information known at the time and may change if your treatment needs change.


When You Will Receive the Estimate

You have the right to receive a Good Faith Estimate when:

  • You schedule a healthcare service at least 3 business days in advance

  • You request an estimate before scheduling services

Best Life Counseling Services will provide a Good Faith Estimate in writing within the timeframes required by federal law.


If Your Final Bill Is Higher Than the Estimate

If you receive a bill that is at least $400 more than the Good Faith Estimate, you have the right to dispute the bill.

You may start a dispute resolution process through the U.S. Department of Health and Human Services.

Keep a Copy of Your Estimate

It is recommended that you keep a copy or photo of your Good Faith Estimate for your records.

This will help if you need to review expected charges or initiate a dispute.
